Who is eligible for Triumph Home Health Care Services?

To quality for our services, a patient must demonstrate the following:

• Be under the care of a physician
• Need skilled nursing and/or therapy care on an intermittent basis
• Meet their specific insurance requirements (Please speak with our intake nurse for further information)

We offer support by providing:

• One-to-one patient care in the home
• Easy referral process
• Emphasis on patient and family education
• Focus on functional outcomes and maximum patient independence
• Ongoing communication with case managers, family and physicians

• In-home assessment to establish a plan of care
• Skilled Nursing
• Physical Therapy
• Occupational Therapy
• Speech Therapy
• Medical Social Workers
• Home Health Aide

What areas of Michigan do you serve?

We serve the greater Metro Detroit area including Wayne, Oakland, Macomb, Livingston, Monroe and Washtenaw counties. We also serve the Lansing area including Shiawassee, Ingham, Eaton and Clinton counties.

How is home health care paid for?

Medicare provides 100% payment for eligible patients.
